Job Title: Community Healthcare Worker
Reports to: Director of Special Services
Summary
This position will focus on addressing social determinants of health and navigating students and families through complex healthcare and needs within the Archie School District area. This includes assisting families with food insecurity, transportation, community referrals, benefits or assistance applications, and addressing a patient’s individual barriers to excellent overall health and wellness.
Community Health Workers (CHWs) are passionate about serving the district students and their community. CHWs strive to understand, motivate, and help our students and families achieve their goals in health and wellness by eliminating barriers, collaborating with families to set goals, and working within school teams. CHWs go above and beyond, often assisting students and families through innovative methods in nontraditional locations. CHWs must be flexible, knowledgeable about community resources, and able to address barriers to care.
Essential Duties and Responsibilities
- Meet students and families in the school setting and conduct needs assessments, and screenings, including identification of socio-economic issues that affect their overall health.
- Assist with assigned outreach activities such as assisting with appointment scheduling, assisting with essential documents and paperwork, attending health fairs, or attending other outreach events, as directed.
- Engage students and families in the development of health and social goals and coach students and families in the effective management of their health conditions and self-care utilizing techniques.
- Build relationships and follow up with students and families about their health.
- Work collaboratively, respectfully, and effectively within school and care teams.
- Address types of barriers to health through activities such as Medicaid enrollment assistance.
- Build and maintain positive, supportive working relationships with students, families, school staff, providers, community organizations, and others.
- Contribute to and utilize data to achieve program goals, improve health outcomes, and advance community health indicators.
- Collaborates with school administrators and community health partners to establish a school based health team model.
- Maintains accurate records for the purpose of assisting students, families, and school teams.
- Attends team meetings to participate in the problem solving model for students and families.
- Maintains confidentiality regarding medical and educational records.
- Maintains a daily log of school/staff visits and documentation.
- Perform other duties as assigned.
Qualifications
Education/Experience
CNA, RN, LPN, BSW, EMT, CHES, Public Health, Community and Family
Services, Health Coach, Case Management, or Social Work experience and/or education.
- Fluent in English.
- Must be 21 years of age or older.
- Valid Driver’s License. Must not have more than two (2) moving violations in the past three (3) years.
- Must possess Basic Life Support certification upon hire or within six (6) months of date of hire.
- Possess and utilize excellent interpersonal skills and professional judgment
- Demonstrate collaborative and consultative skills
Terms of Employment: 10.5 Month (based off Immediate Responsive Services Grant, IRSG)
Evaluation : Performance of the job will be evaluated in accordance with provisions of Board policy.

Part Time Parents as Teachers Position -
The Archie School District is seeking a Parent Educator in the Parents as Teachers Program. The part-time Parent Educator provides parent education, developmental screenings and information as well as daily support to parents through personal home visits to promote the healthy development of children from pregnancy through kindergarten.
Position Qualifications:
- ●Bachelor’s degree in early childhood development or related field is preferred; or-
- ●Minimum of 60 college hours in early childhood development with a minimum of two years of experience directly working with young children under the age of 5 required
- ●Certification in Early Childhood Education, Elementary Education, or 5 years of experience in a preschool setting is preferred.
- ●Required continuing education to maintain certification
Essential Job Functions:
- ●Attend training to become a Certified Parent Educator.
- ●Provide personal home visits to families on assigned caseloads.
- ●Plan and deliver quality-based early learning resources and effective child-parent interaction techniques using the Parents as Teachers (PAT) instructional curriculum-based activities, methods and materials during the day and evening.
- ●Provide early detection and developmental screenings to children zero to five years of age.
- ●Monitor client caseload and record child’s developmental progress.
- ●Maintain client confidentiality and integrity.
- ●Maintain accurate records of all personal home visits, contacts, screenings, resource linkages and data input of progress and required reports.
- ●Participate in community outreach events.
- ●Attend staff meetings.
- ●Maintain 20 hours of required professional development.
- ●Perform all other duties as assigned and adhere to ongoing Archie R-V policies, program standards and supervisory directives.
Salary:
- ●$50 per home visit regular
- ●$0.625 per mile for home visits (from school)
- ●$30 per screening
- ●$25 per hour for group events
